These forms require Adobe Reader. If this application is not already installed on your computer, you may download it free of charge from Adobe.
- Host Family’s Expectations for a Good Relationship - Some tips and advice to hosting a student from a foreign culture.
- Student Permission to Travel - Required form in order to travel. Please process this form at least 2 weeks in advance of the travel request.
- Student Bank Account (W-8) Form - Please have the student use this form to open up a bank account in their own name. For the protection of the student and the host family, ETC requires that ONLY the students have their names associated with their bank account and that no host family member or person(s) other than the student have their names as co-signers, etc.
- Host Family Evaluation - Give us your feedback on your hosting experience.
To complete your background check, first verify with the password verbally with an ETC representative by phoning 503-222-9803. Next, click the appropriate program link below, enter your password, and click "I agree" to log in. All information will remain private; only the last few digits of the social security number are made available to ETC when the results are verified through Intellicorp.
